If you’re a patient at one of our clinics, and you have an urgent need after hours, our on-call practitioner can be reached by calling 360-715-2492.

Can I make an appointment?

Yes, give us a call between 7am and 3:30pm weekdays to schedule an appointment, we are seeing patients from 8am to 5pm. We are following specific infection control protocol that means the visit experience will be a bit different.  When you call to make an appointment you will be asked a series of screening questions to help us decide if it is safe to come in now or if a delay due to exposure to illness is required.

What should I do before the appointment?

Please submit Patient Registration and HIPAA forms through our secure email address HeatherB@Dafo.com before your appointment date. You’ll find the forms here: https://cascadepoclinics.com/patient-resources/registration-forms/

Only one patient (and one caregiver, if needed) will be allowed into the clinic at a time, so please plan accordingly.

All visitors should bring a mask to the appointment.  If you do not have one, we will provide you with one when you arrive.

What will happen during the appointment?

When you arrive, please remain in your vehicle and call this number if you are in Ferndale, 360-543-9244; for the Mt. Vernon office call 360-428-4003. Then put on your mask and wait for our staff to come out to you in the parking lot.

A staff member will come out and screen the patient and caregiver. They will take your temperature(s).  

(If you do not pass the screening, they will help you reschedule for a later date.)

Then they will take a photo of the patient and the patient’s insurance card.

You can request to be seen outside, at your car, depending on the nature of your appointment.  Otherwise, we will ask you to come into the clinic.

When you enter the clinic, you will be taken straight to a freshly sanitized exam room. The staff will ask you to wash your hands.  After the appointment is over, you will be asked to wash your hands again and then exit to your vehicle.

What happens after the appointment?

After each appointment we fully sanitize the exam room.

Instead of scheduling at the desk on your way out, we will call you to schedule any necessary follow up appointments.  And as usual, please call anytime you have a concern or need to come in.
